Located at Peace Arch Hospital in White Rock, BC, as the Nurse Practitioner (NP) for PATH – Patient Assessment and Transition to Home / Community, you play a key role in promoting and implementing an integrated model of primary and community care in multiple practice settings, moving between hospital and community environments.
You will oversee and direct the care of the PATH patient, and facilitate complex transitioning from hospital to home. This work will be done in collaboration with physicians, and where already in place, NPs from the community Division of Family Practice, the PATH interdisciplinary team, and other members of the multidisciplinary community health care team working within Home Health and the Integrated Health Network.
As you follow patients out into the community and ensure their health and wellness is maintained in the transition and that a sustainable health care plan is in place in the home, you will also provide professional guidance to other health professionals and practice autonomously and interdependently within the context of an interdisciplinary health care team. You will focus on continuity of care across the health care continuum, and makes referrals to physicians and others as appropriate, and ensure transition of information at transfer between MRPs.
The NP contributes to a reduction in length of stay on PATH, and a decrease in readmission rates from the provincial average of 27%. The NP may hold a small caseload/practice of frail elderly patients in the community who are home bound and unattached, or home bound and unable to visit their GP in the office. Once these patients are well established in their residence with a plan of care in place, they may be transferred to the care of the Division of Family Practice community based NP.
This position reports directly to the FH Director, Health Services (PATH Program) and will have a matrix reporting relationship with the Division of Family Practice. The NP, Director, Health Services, and the Division of Family Practice will work closely together to support the NP role in its function and mandate. The NP will also report to FH Professional Practice for professional support.
Your qualifications include current practicing registration as a Nurse Practitioner with the College of Registered Nurses of British Columbia (CRNBC) and relevant, clinical nursing experience.
