| We currently have an opportunity for a Case Manager in our Home Health Program located in Maple Ridge/Pitt Meadows, British Columbia. This position offers you the opportunity to: • Provide comprehensive case management to complex geriatric clients and adults with chronic and acute illness • Work with clients/families to address their psychosocial and environmental needs • Establish and maintain relationships through ongoing assessment process • Collaborate and facilitate access to wide range of services To be the successful candidate, you will have: • Graduation from an approved school of Nursing • Current registration with the College of Registered Nurses of British Columbia (CRNBC) • Recent 2 year's nursing experience working with complex geriatric clients and adults with chronic and acute illness • Current BC Driver's License and access to a personal vehicle careers.fraserhealth.ca | facebook.com/fraserhealthcareers |
