In this key role, you are responsible for providing leadership, coordination, facilitation and support in the redesign and implementation of clinical programs in Home Health into Integrated Health Networks (IHN) within Fraser Health. Be part of a dynamic, interdisciplinary team as you work in collaboration with Directors, Managers, Clinical Leaders, clinical teams and committees in the redesign of clinical programs into the Integrated Health Networks using the principles of CARE Management. As the successful candidate, you have a Bachelor of Science degree in a relevant clinical discipline and five years' recent experience in clinical planning, project management, quality improvement methodologies and/or organizational transformation. Your strong understanding of clinical programs, demonstrated facilitation, consultation and presentation skills, and ability to motivate and influence leaders, physicians and staff at all levels are key to your success. The office location for this position is New Westminster with Abbotsford as an alternate location, however, travel throughout the Fraser Health region is required. |
